Default Giving Rights to Graphics?

Hello everyone!

This is my first post on this forum and I'm confident I'll get expectant results.

Well, I am a freelance graphic designer/web builder/PR/Advertising source. I have several clients that I have designed logos, brochures, business cards, web sites, and so on for. It has come to my attention that I know nothing about handing over legal rights to these graphics as I've promised them prior to development.

Does anyone know where I can find some sort of a template that I may modify or use as a reference to sign over the rights of the graphics to my clients? I've tried searching Google, and surprisingly, nothing satisfactory resulted. Even a general rights template or reference I'm sure would be fine!
